Hospice Medical Director
Provides the primary link and involvement at every stage of the ongoing care of the patient.
Registered Nurse / Licensed Vocational Nurse
Skilled nurses who are trained specifically to deal with pain and symptom management available 24 hours a day.
Social Worker
Psychosocial counseling, bereavement services and community referrals.
Certified Home Health Aides and Homemakers
To assist with personal needs.
Chaplains
Available to provide spiritual support if needed to patient families and caregivers.
Volunteers
To provide companionship and assistance through visits, writing letters, legacy building, reading and other activities.
Hospice Services
Skilled Nursing
Licensed nurses provide pain management, symptom control, medication administration, and ongoing clinical assessments.
Medical Social Work
Counseling, community resources, advance care planning, and emotional support for patients and families.
Spiritual Care
Chaplain services and spiritual counseling tailored to each patient's beliefs and wishes.
Bereavement Support
Grief counseling and support groups for families — before, during, and after the loss of a loved one.
Home Health Aide
Assistance with personal care, bathing, grooming, and daily activities — delivered with warmth and care.
Volunteer Support
Trained volunteers provide companionship, respite for caregivers, and help with light tasks and errands.
Who Qualifies for Hospice Care?
- ✓Patient has been diagnosed with a terminal illness with a life expectancy of six months or less
- ✓The Primary Care Physician certifies the patient's eligibility
- ✓The patient and family choose comfort-focused care over curative treatment
- ✓Care can be provided at home, in an assisted living facility, or in a nursing home
